Application

LATEST APPLICATION FORMS CAN BE DOWNLOADED FROM THIS SITE BUT NOT COMPLETED ONLINE

*** PLEASE POST COMPLETED FORMS TO OUR POSTAL ADDRESS ***

Please ensure that you quote either the applicant’s age or date of birth.
Please also total the income and expenditure columns and comment on any surplus or shortfall and abnormal amounts

It will help if you can advise the payees name for our cheque. It cannot be the applicant themselves.

If these points are not covered the appeal may be delayed.

Carpets

We are facing numerous appeals to carpet multi rooms. As a small charity we are unable to consider
carpeting more than one room - this must be the most essential room and dimensions and a quote must
be given to include cost per sq mtr/yd

We will however consider more than one room if the appeal can be justified but each room must be
specified and priced accordingly. We may reduce the award however to one room.

Electrical Goods

All goods are supplied without responsibility of the Trustees or Officials of the Mother Humber Memorial
Fund. The beneficiaries must check any recall notices which manufacturers may from time to time issue.
Tumble Dryers - In view of the continuing concerns regarding the safety of these items and the high
running costs we will not consider appeals for Tumble Dryers until further notice

Beds

Your appeal for beds must state type, size and price together with name of proposed supplier. Appeals for 'special sized or orthopaedic' beds must be accompanied by written support from a practitioner.
